Summary Statement of Deficiencies D5209 PERSONNEL COMPETENCY ASSESSMENT POLICIES CFR(s): 493.1235 As specified in the personnel requirements in subpart M, the laboratory must establish and follow written policies and procedures to assess employee and, if applicable, consultant competency. This STANDARD is not met as evidenced by: Based on a review of the laboratory's competency assessment policies, competency evaluation records and an interview with the laboratory operation manager, the laboratory failed to follow their establish written policies and procedures to assess the competency of the laboratory testing personnel. FINDINGS: The laboratory operation manager confirmed on September 15, 2022 at approximately 11am, that the laboratory did not follow the established competency evaluation policy. The laboratory did not perform 2021 annual competency evaluation for three of ten laboratory testing personnel.
