Summary Statement of Deficiencies D6053 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(9) The technical consultant is responsible for evaluating and documenting the performance of individuals responsible for moderate complexity testing at least semiannually during the first year the individual tests patient specimens. This STANDARD is not met as evidenced by: Based on review of testing personnel competency records and interview with Technical Consultant # 2 and #3, the Technical Consultant (TC) failed to evaluate the performance of 1 of 5 testing personnel responsible for moderate complexity testing of Chemistry, Hematology, Bacteriology, Mycology, and Parasitology specimen, at least semi - annually during the first year. Findings Include: 1. On the date of survey (02/08/2018), review of testing personnels #3 records revealed they were hired on 07 /10/2017 and their competencey assessment was performed on 02/02/2018. 2. Documented on the CMS 116, which was submitted at the time of survey, the total annual volume of patient tests performed was 5,068. 3. TC #2 and #3 confirmed the findings above on 02/08/2018 around 11:00 AM.
