Summary Statement of Deficiencies D1002 REPORTING OF SARS-CoV-2 TEST RESULTS During the Public Health Emergency, as defined in 400.200 of this chapter, each laboratory that performs a test that is intended to detect SARS-CoV-2 or to diagnose a possible case of COVID-19 (hereinafter referred to as a "SARS-CoV-2 test") must report SARS-CoV-2 test results to the Secretary in such form and manner, and at such timing and frequency, as the Secretary may prescribe. This CONDITION is not met as evidenced by: Based on interview with the Senior Clinic Manager and review of clinic records, the laboratory failed to report both positive and negative COVID results to the Oregon Health Authority. Findings include: 1. During an interview with the Senior Clinic Manager at approximately 4:00 pm on January 11, 2022, the Manager confirmed that they are not reporting negative COVID test results to Oregon Health Authority using the Abbott ID Now testing platform. 2. From April 15, 2020 until date of survey January 11, 2022, the clinic performed One Thousand Seven Hundred Thirteen (1713) COVID antigen tests using the Abbott ID NOW test platform. Seventy Five (75) tests were positive and OneThousand Six Hundred Thirty Eight tests (1638) were negative.
