Summary Statement of Deficiencies D0000 An announced recertification survey was conducted on 12/1/21 at Saltzman Tanis Pittell Levin & Jacobson Inc., DBA Pediatric Associates West Palm Beach, a clinical laboratory in West Palm Beach, Florida. Saltzman Tanis Pittell Levin & Jacobson Inc., DBA Pediatric Associates West Palm Beach is not in compliance with Code of Federal Regulations (CFR) 42, Part 493, Laboratory Requirements. The following is a description of the noncompliance. . D5411 TEST SYSTEMS, EQUIPMENT, INSTRUMENTS, REAGENT CFR(s): 493.1252(a) Test systems must be selected by the laboratory. The testing must be performed following the manufacturer's instructions and in a manner that provides test results within the laboratory's stated performance specifications for each test system as determined under 493.1253. This STANDARD is not met as evidenced by: Based on record review, and interview, the laboratory failed to report throat cultures between 18-24 hours per their policy for 82 out 166 patients reported since 1/1/21. The findings included: Review of policy "Strep A Throat Culture" stated to incubate for 18-24 hours.
