Summary Statement of Deficiencies D6053 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(9) The technical consultant is responsible for evaluating and documenting the performance of individuals responsible for moderate complexity testing at least semiannually during the first year the individual tests patient specimens. This STANDARD is not met as evidenced by: Based on the review of training records and interview with the Planned Parenthood of Columbia Willamette (PPCW) Center Manager and the PPCW Manager of Clinical Quality Improvement during the survey 1/21/2020 from 1030 until 1500 (3 pm), the Technical Consultants (TC) at six (6) sites under this CLIA certificate failed to perform written documentation of competency assessment in the first year of employment for new Clinical Assistants (CA's). Findings include: 1. One out of one new CA at the Bend location had no record of initial training, six (6) month competency or twelve (12) month competency for the Eldon Card Rh procedure. 2. Three out of nine new CA at the Beaverton location had no record of initial training or six (6) month competency for the Eldon Card Rh procedure. One out of nine new CA's lacked the TC's signature on the six (6) month competency. 3. One out of three new CA at the NE Marttin Luther King Jr. location had no record of six (6) month competency for the Eldon Card Rh procedure. One out of three new CA's lack the TC's signature on the initial training document. 4. One out of seven new CA at the Milwaukie location had no record of initial training for the Eldon Card Rh procedure. 5. One out of seven new CA at the Milwaukie location had no record of initial training or six (6) month competency for the Eldon Card Rh procedure. 6. One out of seven new CA at the Milwaukie location had no record of six (6) month competency, twelve (12) month competency or annual assessment for the Eldon Card Rh procedure. 7. One out of three new CA at the Salem location had no record of initial training or twelve (12) month competency for the Eldon Card Rh procedure. 8.
