This STANDARD is not met as evidenced by: Based on review of competency/performance evaluations and interview with the program manager , the technical consultant failed to evaluate and document the competency for one of one testing personnel performing moderate complexity testing for 2020. Findings: 1. Review of competency/performance evaluations revealed no documentation to show the technical consultant evaluated and documented the competency for one testing personnel performing moderate complexity testing of KOHs. 2. Interview with Program Manager on December 15, 2021 at 10:00 AM confirmed the technical consultant failed to evaluate and document the competency of all testing personnel for 2020.
