This STANDARD is not met as evidenced by: Based on surveyor observation and staff interview the laboratory failed to use in date vacutainers for blood collection. Findings include: 1. Surveyor observation of the laboratory's blood collection facility supplies on 9/5/18 at 10:30 AM revealed the following vacutainers used for blood collection were expired. a. Six BD vacutainer dark blue top tubes (Lot# 7180594) with an expiration date of 7/31/18. b. Four BD vacutainer gray top tubes (Lot# 7100990) with an expiration date of 8/31/18. 2. Staff interview with the technical consultant on 9/5/18 at 10:30 AM confirmed the above findings.
