Summary Statement of Deficiencies D6127 TECHNICAL SUPERVISOR RESPONSIBILITIES CFR(s): 493.1451(b)(9) The technical supervisor is responsible for evaluating and documenting the performance of individuals responsible for high complexity testing at least semiannually during the first year the individual tests patient specimens. This STANDARD is not met as evidenced by: Based on review of laboratory personnel competency records, and interview with technical supervisor #1 (TS#1), the technical supervisor failed to evaluate and document the performance of individuals responsible for high complexity testing at least semiannually during the first year the individual testing patient specimens hired from August 2016 to February 2017. Findings include: 1. On the date of the survey (01 /30/2018),while reviewing a sample (54 of 92 personnel) of TP competency assessment records it was discovered that the laboratory failed to perform a semiannual competency during the first year of employment for 4 TP. 2. The TS #1 confirmed the findings above on 01/30/2018 around 04:00 PM.
