Summary Statement of Deficiencies D6120 TECHNICAL SUPERVISOR RESPONSIBILITIES CFR(s): 493.1451(b)(7)(8) (7) The technical supervisor is responsible for identifying training needs and assuring that each individual performing tests receives regular in-service training and education appropriate for the type and complexity of the laboratory services performed; (8) Evaluating the competency of all testing personnel and assuring that the staff maintain their competency to perform test procedures and report test results promptly, accurately and proficiently. This STANDARD is not met as evidenced by: Based on review of personnel records and interview with the TS (technical supervisor) 2/15/22, the TS failed to evaluate the competency of 3 of 3 GS (general supervisor) for supervisory duties. Review of personnel records revealed GS #2 was hired in December 2019. Personnel records revealed GS#2 had testing competency evaluations in June and November 2020, and April and October 2021. Personnel records also revealed GS #1 and GS#3 had testing competency evaluations completed in April and October 2019, April and October 2020, and April and October 2021. There was no documentation that the 3 of 3 GS were evaluated for the performance of supervisory responsibilities. During interview at approximately 1:45p.m., the TS (lab director) confirmed that the 3 of 3 GS had not been evaluated for performance of supervisory duties.
