Interview with the Technical Supervisor on October 13, 2026 at 1425 in the laboratory confirmed there was no sink in the laboratory. E. Review of the pre-survey paperwork titled Annual Test Volume & Proficiency Testing Programs Worksheet showed testing began 02/09/2026 (elapsed time 8 months 4 days). Review of the pre- survey paperwork titled CMS Form 116 showed an estimated test volume of 1,706,981 per year in the specialty of Microbiology.
