Summary Statement of Deficiencies D0000 An offsite COVID survey was conducted for Rose's Pharmacy, CLIA #19D2249858 on September 2, 2022. In interview on August 30, 2022 at 012:39 pm, the Contact Person with Rose's Pharmacy stated the facility started patient testing on March 29, 2022 and that a policy was in place for patient testing and reporting results to state agency. He further stated the facility utilizes the BD Veritor Plus system for patient testing. The Contact Person emailed the requested documents for confirmation that the facility is reporting COVID 19 patient test results to the State of Louisiana Department of Health as required by the CMS. The laboratory was found in compliance with 42 CFR 493 Requirements for Laboratories. No deficiencies were cited.
