This STANDARD is not met as evidenced by: Based on record review and interview with testing personnel (TP) A, the laboratory failed to document the annual competency assessment on 1 out of 6 (A, B, C, D, E, F) testing personnel for 2017 Findings include: Review of employee documentation showed that the laboratory failed to have documentation of the annual competence evaluation for the year 2017 for TP E During an interview on 01/24/2019 at 11:00 AM, with the TP A, he confirmed that there was no annual competency assessment documented for TP E for the period of reference.
