Summary Statement of Deficiencies D3011 FACILITIES CFR(s): 493.1101(d) Safety procedures must be established, accessible, and observed to ensure protection from physical, chemical, biochemical, and electrical hazards, and biohazardous materials. This STANDARD is not met as evidenced by: Based on record review and interview with the Laboratory Manager, the laboratory failed to dispose of chemicals per manufacturer's instructions for 2 out of 2 years (2017-2018) reviewed. Findings Include: Review of the manufacturer's instructions on the bottles of alcohol, eosin, and xylene substitute all state that they should be disposed of in a waste disposal plant. During an interview on 11/15/18 at 11:00 AM' the Laboratory Manager confirmed that the alcohol, eosin, and xylene substitute are disposed of down the sink drain.
