Summary Statement of Deficiencies D0000 A recertification survey was conducted on December 6, 2018. Skin Dermatology and Cosmetic Services PA had one deficiency found at the time of the visit. D5417 TEST SYSTEMS, EQUIPMENT, INSTRUMENTS, REAGENT CFR(s): 493.1252(d) Reagents, solutions, culture media, control materials, calibration materials, and other supplies must not be used when they have exceeded their expiration date, have deteriorated, or are of substandard quality. This STANDARD is not met as evidenced by: Based on observation and laboratory coordinator interview, the laboratory had expired staining reagents used for staining pathology slides. The findings included: During the laboratory walk through on observation, it was determined that three bottles of Quick- Dip Stains were found stored in containers kept inside the chemical cabinet and expired 6/11/17. The interview with the laboratory coordinator on 12/5/18 at 10:30 AM confirmed that expired reagent stains were located in the laboratory and was unsure if the stains were currently being used.
