Summary Statement of Deficiencies D6127 TECHNICAL SUPERVISOR RESPONSIBILITIES CFR(s): 493.1451(b)(9) The technical supervisor is responsible for evaluating and documenting the performance of individuals responsible for high complexity testing at least semiannually during the first year the individual tests patient specimens. This STANDARD is not met as evidenced by: Based on review of the laboratory's competency assessment records and interview with testing personnel #1 (TP), the technical supervisor (TS) failed to evaluate the semiannual competency assessment for 3 of 3 TP who performed virology examinations from April 2022 to the day of the survey. Findings include: 1. On the day of survey, 02/28/2023 at 09:10 am, the laboratory could not provide semiannual competency assessment records for 3 of 3 TP (CMS 209 personnel #2, #3, and #4) who performed SARS-Cov-2 real time polymerase chain reaction examinations from April 2022 to the day of survey. 2. TP #1 confirmed the findings above on 02/28/2023 around 11:00 am.
