Summary Statement of Deficiencies D0000 An announced CLIA recertification survey was conducted at Stuart Oncology Associates PA on February 24, 2026 to April 28, 2026. The laboratory was surveyed under 42 CFR Part 493 CLIA requirements. Standard deficiencies cited are as follows: D5209 PERSONNEL COMPETENCY ASSESSMENT POLICIES CFR(s): 493.1235 As specified in the personnel requirements in subpart M, the laboratory must establish and follow written policies and procedures to assess employee and, if applicable, consultant competency. This STANDARD is not met as evidenced by: Based on interview, and record review the laboratory failed to have an initial competency assessment for Technical Consultant A in 2025. Findings Included: Review of Competency Assessments revealed there was no competency in 2025 for Technical Consultant A. Review of Staff Orientation, Training, and Competency Policy revealed no policy on competency assessments for Technical Consultants. On 2 /24/26 at 5:15 PM, Technical Consultant A stated there was no competency for them in 2025 and there was no policy for competency assessments for Technical Consultants.
