Summary Statement of Deficiencies D5209 PERSONNEL COMPETENCY ASSESSMENT POLICIES CFR(s): 493.1235 As specified in the personnel requirements in subpart M, the laboratory must establish and follow written policies and procedures to assess employee and, if applicable, consultant competency. This STANDARD is not met as evidenced by: Based upon record review and interview with the Laboratory Manager, the laboratory failed to perform a competency evaluation for 1 out of 1 (#A) testing personnel for 2 out of 2 years (2016-2018). Findings included: At the time of survey, competency records for Testing Personnel #A were requested but the laboratory was unable to provide the competency records for 2 out of 2 years (2016-2018). The procedure manual for the laboratory did not contain a procedure for evaluating competency of testing personnel. During the interview on 03/07/18 at 12:30 p.m., the Laboratory Manager confirmed the laboratory did not have a competency procedure for the survey time period ( 2016-2018) and did not perform the competency evaluations for Testing Personnel #A.
