Summary Statement of Deficiencies D5209 PERSONNEL COMPETENCY ASSESSMENT POLICIES CFR(s): 493.1235 As specified in the personnel requirements in subpart M, the laboratory must establish and follow written policies and procedures to assess employee and, if applicable, consultant competency. This STANDARD is not met as evidenced by: Based on record review and interview with the Laboratory Director, the laboratory failed to perform competency assessment for one (#B) of two Testing Personnel. Findings included: Review of the CMS 209 Laboratory Personnel Report revealed Personnel #B held the position of Testing Person. During the laboratory record review it was found that documentation was missing for competency assessment for Testing Person #B. During an interview on 05/10/2018 at 12:00 PM, the Laboratory Director confirmed that competency assessment had not been performed for Testing Person #B.
