Summary Statement of Deficiencies D6054 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(9) The technical consultant is responsible for evaluating and documenting the performance of individuals responsible for moderate complexity testing at least annually, after the first year. This STANDARD is not met as evidenced by: Based on review of the annual evaluations and interview with the testing person, the technical consultant did not ensure that all testing personnel received an annual competency review. Findings: 1. The laboratory currently has one testing persons listed on the "Laboratory Personnel Report (CLIA) (CMS-209)." The laboratory director is acting as the technical consultant. 2. There were no evaluation records for 2017 and 2018 showing that the testing personnel were evaluated annually. 3. During the survey on 10/16/2018 at 10:30 AM the testing person confirmed that the annual evaluations for 2017 and 2018 were not performed by the laboratory director.
