The procedures for evaluation of the competency of the staff must include, but are not limited to-- This STANDARD is not met as evidenced by: _ Based on lack of documentation, and interview with Testing Personnel 1 (TP1), the laboratory failed to evaluate the competency of 2 of 2 testing personnel performing qualitative semen analysis. The laboratory performs approximately 3,500 tests a year. _ Findings include: 1. Record review revealed the laboratory failed to document and perform competency assessment for 2 of 2 testing personnel performing qualitative semen analysis. 2. In an interview on 4/38/2025 at approximately 2:06 PM, TP1 confirmed that the laboratory failed to document and perform competency assessment for 2 of 2 testing personnel.
