• Search Functionality: Search and filter CMS-2567 deficiency reports by state, date range, facility name, CMS certification number, deficiency tags, and other criteria
• Map Visualizations: Interactive maps displaying facility locations and citation data
• Facility Pages: Dedicated pages for each facility with detailed citation history
• CSV Downloads: Purchasable CSV exports of survey data for specific states, years, and months
• Premium Features: Access to detailed survey information, lab director names, deficiency summaries, and PDF reports
• Reports Dashboard: Summary statistics and facility demographics

3. Use License and Permitted Uses

Subject to these Terms, CLIA Lab Citations grants you a limited, non-exclusive, non-transferable license to access and use the Service for lawful purposes. You may:

• Search and view publicly available citation data
• Use purchased CSV downloads for your business or research purposes
• Access premium features for which you have paid
• Share links to facility pages and search results

You may NOT:

• Scrape, crawl, or use automated systems to extract data from the Service without explicit written permission
• Resell, redistribute, or republish purchased CSV downloads to third parties
• Modify, reverse engineer, or attempt to extract the source code of the Service
• Use the Service to create a competing service or product
• Remove copyright notices, watermarks, or attribution from materials
• Use the Service in any way that violates applicable laws or regulations
• Share your account credentials or purchased content access with others

This license is revocable at any time and will automatically terminate if you violate these Terms.

4. Data Source and Accuracy

The data provided through this Service is sourced from publicly available CMS-2567 reports published by the Centers for Medicare & Medicaid Services (CMS). This includes:

• Survey dates, types, and event IDs
• Facility names, addresses, and contact information
• CMS certification numbers and CLIA IDs
• Deficiency tags and summaries
• Lab director information (when available in public records)

• We make no warranties or representations regarding the accuracy, completeness, timeliness, or reliability of the data
• Data is provided "as is" without warranty of any kind, express or implied

• Data availability depends on CMS publication schedules and may not include all recent surveys

5. Payment Terms and Purchases

Certain features of the Service require payment, including:

• Premium Search Access: Full access to detailed survey data, lab director information, and PDF reports ($159.00 one-time payment)
• CSV Downloads: State-specific, monthly survey data exports ($99.00 per state per month)
• D-tag Reports: Specialized deficiency analysis reports ($59.00 per lab, optional add-on)
• Facility Unlocks: Access to individual facility PDF reports and summaries ($29.00 per facility or $19.00 per citation)

By making a purchase, you agree to:

• Pay all fees associated with your purchase at the time of purchase
• Provide accurate payment information through our payment processor (Stripe)
• Authorize us to charge your payment method for the applicable fees
• Understand that prices are subject to change without notice
• Accept that all sales are final unless otherwise stated

Refund Policy:

• All sales are final. Refunds may be provided at our sole discretion in exceptional circumstances (e.g., technical errors preventing access, duplicate charges)
• Refund requests must be submitted within 30 days of purchase
• Refunds will be processed to the original payment method within 5-10 business days
• No refunds will be provided for downloaded CSV files or accessed premium content

Payment Processing: Payments are processed securely through Stripe. Your payment information is handled in accordance with Stripe's privacy policy and security standards. We do not store your full credit card information on our servers.

6. Access and Session Management

The Service uses browser-based session management (localStorage and sessionStorage) to track your access status and purchases. You are responsible for:

• Maintaining the security of your device and browser
• Not sharing your device or browser session with unauthorized users
• Understanding that clearing your browser data may revoke your access to purchased content
• All activities that occur using your browser session
• Notifying us immediately if you suspect unauthorized access to your purchased content

Note: Access to premium features and purchased downloads is tied to your browser session. If you clear your browser data, you may need to contact support to restore access to your purchases.

7. Prohibited Uses

You agree NOT to use the Service:

• In any way that violates any applicable federal, state, local, or international law or regulation
• To transmit, upload, or introduce any malicious code, viruses, trojans, worms, or harmful data

• To reverse engineer, decompile, or disassemble any software or technology used in the Service
• To use the Service to create a competing product or service
• To impersonate any person or entity or misrepresent your affiliation with any person or entity
• To collect or harvest personal information about other users
• To use the Service for any illegal, fraudulent, or harmful purpose
• To circumvent or attempt to circumvent payment mechanisms or access controls

Violation of these prohibitions may result in immediate termination of your access, legal action, and reporting to law enforcement authorities.

8. Intellectual Property Rights

Service Ownership: The Service, including its design, features, functionality, code, graphics, logos, and user interface, is owned by CLIA Lab Citations and protected by copyright, trademark, and other intellectual property laws.

CMS Data: The underlying CMS-2567 data is in the public domain as published by the Centers for Medicare & Medicaid Services. However, our compilation, organization, presentation, analysis, geocoding, classification, and additional processing of this data are proprietary to CLIA Lab Citations.

Your Rights:

• You may use purchased CSV downloads for your own business or research purposes
• You may not resell, redistribute, or republish purchased CSV downloads to third parties
• You may not copy, modify, or create derivative works from the Service's interface, design, or functionality
• You retain ownership of any content you create using data from the Service, but you may not reproduce the Service itself

9. Disclaimers
